Two decades of hell made him see everyone and everything as a threat. His mind automatically focuses on the dangerous and unnatural aspects of someone. It's like fight or flight turned up to 11.
With angel here, he doesn't even register the actually nice and, dare I say, beautiful bits. All he sees is the screwed up bits. The multiple limbs, the pale fur, the sharp teeth, the many eyes ETC.

I can relate. Whenever I see a female as curvy/boobie and attractive as Silvervale or Toxicsoul77 for example, I actually see a literal evil monstrosity straight from hell instead. A monsters that better die before they kill anyone.
I almost became one because of curvy/boobie girls like Toxicsoul77. Actually, I want to have a girlfriend, but due to underserved hell I went through, I'll never trust a girl with uncommonly curvy body. Besides, Incels are degenerates like Elliot Rodger and Alec Minassian while I am just a moral wreck who once was framed for a serious crime I'd never really do. Stalking. Stop musunderstanding the shits!
Regardless of the fact that knowing this about him is very interesting, I must say that I feel a little sorry for him because of this.
Not really no. The guy has been trapped in HELL for twenty years, its clear his mental perception is utterly warped. It's how HE sees Angel Dust and not how AD actually looks.
Only 'twenty years'. Haven't AD and the rest been down there a good while in comparison? Wouldn't their perception, as well as that of the viewer, see that space in the native perception, while he is a newcomber and sees things as they are? Metaphysical acclimatization.
I acually drew inspiration from Victor Zsasz and how he sees people. XD
Of course Angel Dust is as dangerous as he sees him, just not for any of the reasons he sees, which could lead to him missing the real dangers Of course if he avoids him entirely it doesn't matter, but still ironic
